Fill out DS-64, Statement for Lost or Stolen Passport and print one copy. Sign in black ink only. The original must be included in the sealed enveloped, the copy goes on the outside with your Texas Tower authorization form.

familiar with passport photo requirements such as Walgreens, CVS, or Kinko's

Proof of Identification

Make a clear copy front and back of a state-issued or federal-issued I.D. Examples can be a valid state drivers license, military I.D., or government I.D. Must be issued more than 6 months ago.

One of the following :

Original U.S. issued Certified birth certificate

Original U.S. Naturalization certificate
